All six United States crew members aboard a military refueling aircraft have been confirmed dead after a mid-air collision over western Iraq, marking one of the deadliest aviation incidents involving U.S. forces since the escalation of the Iran conflict earlier this year.
The United States Central Command (CENTCOM) confirmed on Friday that the KC-135 Stratotanker aircraft crashed during a mission in “friendly airspace,” killing everyone on board. The aircraft was part of ongoing military operations supporting U.S. and allied forces in the region.
Mid-Air Incident During Military Mission
According to military officials, the tragedy occurred on March 12, 2026, when two KC-135 refueling aircraft were conducting aerial operations over western Iraq. During the mission, an incident occurred between the two planes, resulting in one aircraft crashing while the other managed to land safely.
Authorities emphasized that the crash was not caused by hostile or friendly fire, and the circumstances surrounding the incident are now under investigation by U.S. military officials.
The KC-135 Stratotanker plays a critical role in U.S. military operations by refueling fighter jets and bombers mid-air, allowing them to remain in combat zones for extended periods.
Rising Casualties in the Iran-Linked Conflict
The deaths of the six crew members add to the growing toll of the ongoing military confrontation involving Iran. Since the start of the campaign in late February, at least 13 U.S. service members have been reported killed, while more than a hundred others have sustained injuries during operations across the Middle East.
Officials have not yet released the names of the victims, as the U.S. military first seeks to notify their families.
Meanwhile, some Iran-aligned militia groups operating in Iraq have claimed responsibility for downing the aircraft. However, U.S. officials have not confirmed any hostile involvement, maintaining that the crash occurred during operational maneuvers and remains under investigation.

Military analysts say aerial refueling operations are among the most technically demanding maneuvers in aviation, particularly during high-tempo combat missions. Investigators are now examining flight data, communication records, and wreckage to determine the exact cause of the collision.
